IRIS Scorecards

The IRIS consumer scorecards are tools you can use to help you choose an IRIS Consultant Agency (ICA) and Fiscal Employer Agent (FEA). Talk to your Aging and Disability Resource Center (ADRC) to discuss your ICA and FEA selections.

To Request Participant Documents

To request a copy of your IRIS record for yourself or your guardian, please complete this form: Request for Access to Records (Records Request)

If you would like TMG to release your records to someone other than yourself or guardian, a Release of Information form also needs to be completed along with the Records Request form. Your IRIS Consultant can provide you with the Release of Information form or you can call Quality Services at 844-864-8987.
