Introducing your new one-stop shop for all Molina and My Choice credentialing needs



In an ongoing effort to make processes easier for you and your staff, effective immediately, we are consolidating the Credentialing applications, process, and status between Molina Healthcare of Wisconsin, Inc. and My Choice Wisconsin for all lines of business.

(This includes Medicaid SSI, BadgerCare Plus, Medicare Dual-Advantage, Marketplace, Family Care, and Family Care Partnership.)

This means you only need to keep track of one set of applications! You can return all applications or ask all questions to our central team at MHWIProviderNetworkManagement@Molinahealthcare.com. Then, please watch for all correspondence to come from either ‘@MolinaHealthcare.com’ or “@mychoicewi.org’ email addresses.


Provider Action Needed:

To add a new provider or facility to your group for Molina or My Choice, you must complete the appropriate application:

Practitioners – CAQH Checklist
If you are not registered with CAQH, you must complete the Practitioner Application.

Facilities – Health Delivery Organization (HDO) Application
This includes LTSS / LTC facilities.
Note for Molina providers: This form has recently been updated.
Note for My Choice LTSS residential and ancillary providers: There is an additional short addendum on MIDAS.

Send all credentialing applications, regardless of line of business participation, to MHWIProviderNetworkManagement@Molinahealthcare.com.

To check the status of a credentialing application, or if you have any questions, reach out to MHWIProviderNetworkManagement@MolinaHealthcare.com, regardless of the participating line of business.

To add, term, or update the existing demographic for a provider or facility, complete the Provider Information Update form.
