To keep your health plan, you MUST renew your Medicaid benefits every year!

We want you to keep your family in the Healthy Connection Programs. To do so, you have to make sure you renew your Medicaid benefits every year.
When you have been a member of the Medicaid program, the South Carolina Department of Health and Human Services (SCDHHS) will check to see if you still qualify for Medicaid. You will get a package from them asking you for some information. Put this package in a safe place. DO NOT THROW IT AWAY. It is important that you complete the necessary information. When you are done make sure to send it back to SCDHHS as soon as you can. If you don’t want to fill out the form and mail it, you also can go online and choose your health plan by going to www.SCchoices.com.
If you do not do this, SCDHHS will end Medicaid for you and your family.
When SCDHHS makes the decision that you qualify for Medicaid they will send you Healthy Connections Choices Annual Right to Change Packet. The Annual Right to Change letter will say it has been one year since you joined the Molina Healthcare. If you want to keep being part of Molina Healthcare you do not have to do a thing.
