Getting rewarded is easy to do! Here's how:

  1. Get your postage-paid postcard kit in the mail.
  2. Complete your visit.
  3. Tell us about your visit. Fill out your postcard and drop it in the mail. OR, complete the reward form online at MyMolina.com or in the My Molina app. Click on "My Wellness" and complete the "Attestation Form."
  4. We will confirm your visit through claims.
  5. Get your gift card reward in the mail. Keep this card. We will add new rewards on this same card when you earn them.

  • Current Molina members: For going to your first prenatal visit in your first trimester.
  • Newly enrolled Molina members: For going to your prenatal visit within 42 days of joining Molina Healthcare.

  • For going to your postpartum visit 7 to 84 days after your baby is born. (Does not include staple removal for C-sections.)
