Continuity of Care (CoC)

The CoC Policy for the 2024 Managed Care Plan (MCP) Transition provides guidance to MCPs and their Subcontractors about their obligations to ensure continuity of care for members required to change MCPs on January 1, 2024. The policy guide refers to a set of coordination policies designed to safeguard members’ access to care during the 2024 MCP Transition. Please note that CoC is applicable to Out-Of-Network (OON) providers.

  • These policies help to ensure members maintain trusted relationships with providers and access to necessary services as they transition between MCPs.
  • CoC for Providers enables transitioning members to continue receiving care from their existing providers for 12 months (notwithstanding exceptions) and if certain requirements are met.
  • CoC protections extend to Primary Care Providers, Specialists, and select ancillary Providers, including physical therapy, occupational therapy, respiratory therapy, BHT, and speech therapy Providers. CoC protections do not extend to all other ancillary Providers such as radiology, laboratory, dialysis centers, Non-Emergency Medical Transportation (NEMT), Non-Medical Transportation (NMT), other ancillary services, and non-enrolled Medi-Cal Providers.


However, transitioning members who have services in place with a contracted provider are considered for continuation of services. Transitioning members can continue seeing the contracted provider without any further action required for the first six months.

If Molina has received TAR data from DHCS, that TAR will be honored under CoC and no new request is required. To ensure Molina has all of your members’ TARs, please submit your TARs on an Excel spreadsheet. See below for Excel data elements.
