Why Become an IRIS Vendor

The IRIS program - Wisconsin’s fully self-directed Medicaid waiver program - is used by more than 27,000 individuals with disabilities and older adults across all 72 counties in the state. People in IRIS choose the supports and services they need, as well as the service providers who supply those resources.

As an IRIS vendor, you become eligible to provide people in IRIS with the supports and services they need to remain safe, independent, and in the homes they choose. You can rest assured knowing that your services are IRIS-approved, and expand your reach by having your name listed in the public-facing ForwardHealth Provider Directory once you’ve applied on the portal.

You also will receive vendor training and ongoing support from both the Fiscal Employer Agent (FEA) and TMG to help you navigate the IRIS program. How to Become an IRIS Vendor

  1. To begin the set-up process, you will need to apply as a long-term care (LTC) Medicaid Provider with the Wisconsin Department of Health Services (DHS). DHS has a LTC Provider Enrollment website with information on how to do this with videos, written materials and upcoming live trainings. Do note that this part of the application process will take 10 to 60 days, depending on the services you provide, the credentials needed, and if any follow-up is required after completing the application.
  2. During the application period, if you have questions about the Provider Type, Provider Specialty or Waiver Service, you can contact ltcproviderenrollment@dhs.wisconsin.gov. You can also find links to other DHS provider resources in the DHS Resources section of this page.
  3. Once you have received a Medicaid Management Information System (MMIS) ID number from DHS, please email us at TMGVendorSupport@tmgwisconsin.com. We will provide you with the paperwork required to be paid by the Fiscal Employer Agency (FEA).
  4. Return the forms to TMGVendorSupport@tmgwisconsin.com. We will then work with the FEAs to complete the application. Once the application is complete, the FEA will contact you to let you know you are approved.
  5. You can begin to work with the person and their IRIS Consultant (IC) to authorize services.

TMG’s IRIS Vendor Support and Orientation

TMG offers vendor education that connects you with our team so that you learn about the IRIS program, complete the necessary paperwork, and get the education and resources you need to properly support the people enrolled in IRIS.

TMG is also here to provide ongoing support to vendors. If you run into issues such as unpaid claims, rate changes, credentials or service discrepancies, reach out to us and we can help walk you through these obstacles.

DHS Provider Application Information and Resources

The Wisconsin Department of Health Services has a list of resources that can help you during the application process, as well as a digital newsletter that will keep you updated on changes to the IRIS program and how that might affect providers.
